To determine if a patient is severely dehydrated, we look at the patient's creatinine, sodium, and BUN levels. These typically reveal how dehydrated the patient is even if other records in the chart suggest the patient is being hydrated. Malnutrition

While malnutrition cases review a patient's albumin levels, dieticians also examine evidence of vitamin deficiency in a patient's skin, hair, and nail beds. In addition, dieticians look at muscle wasting, weight loss, and the daily meal records. If these records don't align or your loved one’s condition doesn’t justify significant weight loss, there could be a case for malnutrition.
